Who to call: To register at any of the schools in Middletown School District, call the district office at 401-849-2122 to make an appointment.
- A completed student registration form (for grades 1-12) or family registration form (for kindergarten) needs to be submitted along with the child's birth certificate, residency proof, immunization record and parents photo ID.
